Here is some important information to help you prepare for your cancer care appointment.
What to Bring to Your Cancer Care Appointment
Please bring the following to your appointment:
- Photo ID
- Insurance card(s)
- Co-payment, if needed
- Actual prescription bottles of all medications, including vitamins and supplements
Appointment Cancellation Policy
As a courtesy to others who may require our services, please call our office with 24-hour advance notice if you need to cancel your appointment.
Referrals from Another Physician
You are responsible for ensuring that a visit to our physician is covered by your health insurance plan and or is in-network.
A referral from another physician is needed for a new patient appointment with our physician. When your primary care or other physician contacts us, we will provide that office with our Referral Form, which they can fax back to us along with medical notes, lab, pathology and imaging results. Our physicians will review everything, and our office will call you to schedule your appointment.
After-Hours Medical Care
If you have a life-threatening emergency, call 9-1-1 or go to the nearest hospital emergency department.
