Welcome to OhioHealth Cancer Physicians!

Here is some important information to help you prepare for your cancer care appointment or medical care.

What to Bring to Your Cancer Care Appointment

Please bring the following to your appointment:

  • Photo ID
  • Insurance card(s)
  • Co-payment, if needed
  • Actual prescription bottles of all medications, including vitamins and supplements

Appointment Cancellation Policy

We make every effort to schedule an appointment at a time that is convenient for you. As a courtesy to others who may require our services, please call our office with 24-hour advance notice if you need to cancel your appointment.

Referrals from Another Physician

You are responsible for ensuring that a visit to our physician is covered by your health insurance plan and or is in-network.

A referral from another physician is needed for a new patient appointment with our physician. When your primary care or other physician contacts us, we will provide that office with our Referral Form, which they can fax back to us along with medical notes, lab, pathology and imaging results. Our physicians will review everything, and our office will call you to schedule your appointment.

Prescription Refill Policy

First, please check with your pharmacy to see if you have additional refills remaining on your prescriptions. If you believe your prescription will run out before your next office visit, you may call our office to discuss your need for a refill. If you are due for a follow-up appointment before a refill, we will schedule your next appointment. Prescriptions will not be refilled after office hours or on weekends.

After-Hours Medical Care

If you have a life-threatening emergency, call 9-1-1 or go to the nearest hospital Emergency Department.
